In fact, if you add up the total deaths of the 3 diseases that follow heart disease, it stll does not total what ASCVD does.

In order to "banish" it from any population, they would need to eat a high fat diet for multiple generations from childhood. But wait until their 50's to have children, so people who got CA died without passing on the bad genes. Only those with good genes would have children with less chance of CA.

Otherwise It depends on diet on lifestyle.

Happy/contented people with great diet and exercise habits who don't put toxins in their bodies are very unlikely to get CA.
