At what stage is a liver biopsy required for a patient with Hepatitis C?

nutshell = 38yr old female, contracted HCV around 15 years ago related drug use (no drugs anymore) but has been drinking about 1/2 bottle wine per night. I have been told that you should not even look at alcohol if you have HCV
What are the signs?, she does bruise easily but im not sure what else to look for.
She needs a liver biopsy based on results from her previous test. is it bad?

she refuses treatment

Whether or not it is bad depends on if she can afford interferon :)
